Make a Referral
Our participant pre-screening process ensures Xyston is the right fit before any commitments are made. The process is thorough, confidential, and designed to set everyone up for success.
Before You Begin
The assessment takes approximately 10–15 minutes across 7 sections. Have the following documents ready before starting:
- Current NDIS Plan or Plan Summary (required)
- Budget statement showing Support Coordination funding (required)
- Medical or Allied Health reports
- Safety or Behaviour Support Plans (if applicable)
- Guardianship Orders (if applicable)
Accepted formats: PDF, DOCX, JPG — max 10 MB per file

What the Form Covers
Seven sections covering everything we need to assess suitability and prepare the best possible support from the start.

Participant Information
Full name, date of birth, contact details, NDIS number, and primary contact person.

NDIS Plan & Goals
Plan dates, funded support areas, remaining budget, plan document upload, and participant goals.

Holistic Support Needs
Primary disability, medical reports, current supports in place, and barriers to achieving goals.

Initial Risk Considerations
Known risks, restrictive practices history, and any existing safety or behaviour support plans.

Legal & Consent
Guardian or Power of Attorney details, confidentiality agreement, and conflict-of-interest disclosure.

Communication & Culture
Preferred communication methods, language, and any cultural considerations to be aware of.

Declaration
Confirmation of accuracy and electronic signature (type full name).
