Refer a client to Xyston

We accept referrals from GPs, allied health professionals, NDIS planners, Local Area Coordinators, legal guardians, and administrators. Xyston is a registered NDIS provider delivering Coordination of Supports (Level 2) across Perth and Western Australia.

Who can refer?

We accept referrals from any professional or person supporting an NDIS participant who requires a Support Coordinator.

General Practitioners

You've identified a patient with disability-related support needs navigating the NDIS. Refer directly — we'll make contact and manage the coordination from there.

Allied Health Professionals

Occupational therapists, physiotherapists, speech pathologists, social workers, psychologists, and other allied health practitioners referring clients with active NDIS plans.

NDIS Planners & LACs

Referring directly from a planning meeting or connecting a participant who needs ongoing coordination support. We can accept warm handovers and work within the participant's plan budget.

Hospital Discharge Teams

Coordinating discharge planning for a patient with an NDIS plan? We work with hospital social work teams to ensure continuity of support in the community.

Legal Guardians & Administrators

If you hold guardianship or administration orders for a person with a disability, you can refer and engage on their behalf. We're experienced in working within guardianship frameworks.

Community Organisations

Community service workers, mental health case managers, aged care workers, and other support networks referring individuals who have transitioned to, or are accessing, the NDIS.

What we do

Coordination of Supports — Level 2

As a registered NDIS Support Coordinator, Xyston works directly with participants to implement their NDIS plan — connecting them with providers, building capacity, and monitoring their plan to ensure funding is used effectively.

  • Implementing supports identified in the participant's NDIS plan
  • Sourcing and coordinating service providers across all support categories
  • Monitoring plan budgets and flagging underspend or overspend
  • Preparing for plan reviews with supporting evidence
  • Crisis support and brokering urgent service arrangements
  • Building the participant's capacity to self-direct over time
  • Liaising with guardians, administrators, and legal representatives
  • Working within complex or high-needs support environments

We support participants across the Perth metropolitan area and regional WA. If you're unsure whether we service your client's location, contact us before submitting.

Plan-funded — no cost to referrers

Support Coordination is funded through the participant's NDIS plan under Capacity Building — Support Coordination (07_002). There is no cost to the referring professional or organisation.

How the referral process works

From submission to first contact, here's what happens after you refer.

Submit the referral

Complete our online referral form with the participant's details, NDIS plan information, and support needs. You'll receive an email confirmation immediately.

We review within 1 business day

Our team reviews the referral for fit and capacity. If we have a concern or need clarification, we'll contact you directly.

Initial contact with participant

We reach out to the participant (or their nominee) to introduce ourselves, explain our role, and confirm they wish to proceed.

Service agreement and onboarding

Once the participant agrees, we complete a service agreement and begin implementing supports. You'll receive a confirmation when the participant is onboarded.

What to include

The more information you provide, the faster we can confirm capacity and begin onboarding. The referral form will guide you through each section, but as a checklist:

Participant's full name, date of birth, and contact details
NDIS number (if available)
Plan dates and whether the plan is active
Plan management type (agency, plan-managed, or self-managed)
Support Coordination funding confirmed in the plan
Primary disability or diagnosis (if the participant consents to sharing)
Current living situation and key support needs
Any known risks, safety concerns, or complex circumstances
Nominee, guardian, or administrator details (if applicable)
Your contact details and role

About NDIS Support Coordination

Support Coordination (Level 2 — Coordination of Supports) is an NDIS funded support that helps participants implement their plan, coordinate services, and build the skills to manage their own supports over time. It is funded under Capacity Building in the participant's plan and does not reduce funding in other support categories.

A Support Coordinator is not a case manager, but works closely with allied health, medical, and community professionals to ensure the participant's plan reflects their current needs and is being used effectively.

Referring on behalf of a participant under guardianship or administration

If you are a legal guardian, financial administrator, or hold an enduring power of attorney for an NDIS participant, you can refer on their behalf. Please indicate your authority type and provide your contact details in the referral form. We are experienced working within guardianship and administration frameworks in Western Australia and will liaise with you appropriately throughout the engagement.
