Coordination of Supports

Ongoing coordination across your whole support network

Xyston's dedicated coordinators take an active, hands-on role in managing your entire support network — working directly with your providers, resolving issues, preparing you for plan reviews, and steadily building your capacity to navigate the NDIS with confidence.

Everything your coordinator handles

Your Xyston coordinator is not just a point of contact — they're an active participant in your NDIS journey, working across every aspect of your plan.

01

Provider Network Management

We find, connect, and manage all your NDIS providers — taking the administration burden off you entirely.

02

Regular Reviews & Goal Tracking

Scheduled check-ins to review your progress, adjust your supports, and ensure your plan is delivering results.

03

Plan Review Preparation

Comprehensive report writing, evidence collation, and advocacy support ahead of your NDIS plan review.

04

Budget Monitoring & Reporting

We monitor how your funding is being used, flag shortfalls early, and submit required reports to the NDIA.

05

Crisis Planning & Contingency Coordination

We help you build a personalised crisis plan before things escalate — identifying warning signs, key contacts, and steps so you're prepared. After any acute event, we stabilise and rebuild your supports.

06

Capacity Building

Every interaction is designed to grow your confidence and skills so you can increasingly self-direct your NDIS plan.

07

Allied Health Liaison

We coordinate seamlessly with your OT, psychologist, GP, and specialist services to ensure everyone is aligned.

08

Community & Informal Supports

We identify and connect you with community groups, volunteer services, and informal supports that complement your funded plan.

09

Service Gap Resolution

When providers leave, waitlists are long, or funding is insufficient, we advocate and problem-solve on your behalf.

10

Cross-Sector Coordination

We navigate housing, mental health, justice, and health systems on your behalf when multiple services are involved.

Day-to-Day Coordination

An active partner, not a passive contact.

Many participants have had the experience of a coordinator who's hard to reach, rarely proactive, and only gets involved when something goes wrong. At Xyston, we work differently.

Your coordinator regularly checks in with you and your providers, tracks your plan progress, and deals with problems before they escalate. They attend meetings with you, write reports to the NDIA, and prepare you thoroughly for plan reviews — so you're not walking in cold.

Over time, as your confidence grows, we gradually step back — transferring skills and knowledge so you're in control of your own NDIS journey. That's what capacity building actually means in practice.

Who It's Best Suited To

Participants with multiple providers, complex support needs, or those who want a dedicated, experienced coordinator actively managing and improving their NDIS journey.

  • Participants new to the NDIS who need guidance from the start
  • Those transitioning from another coordinator or provider
  • Participants with multiple services across health, disability, and community sectors
  • People whose situation has changed significantly since their last plan review
  • Carers and families seeking structured, expert support navigation

Pricing

Xyston charges the NDIS published price limits — the maximum hourly rates set annually by the NDIA. There are no hidden fees. Your coordinator will discuss your available budget before any service begins.

Questions & Answers

Frequently Asked Questions

Common questions about NDIS Support Coordination and how Xyston works.

Support Coordination (Level 2, item code 07_002) is funded under the Capacity Building category of your NDIS plan. It is a separate budget to your Core Supports. If you don't currently have Support Coordination in your plan, you can request it to be included at your next plan review by speaking with your NDIA planner or Local Area Coordinator (LAC).
A Plan Manager handles the financial side — paying your invoices and managing your budget. A Support Coordinator focuses on the people and services side — finding the right providers, coordinating your support network, and building your capacity. Many participants have both, and they work together.
Yes — you have the right to change Support Coordination providers at any time, subject to your Service Agreement notice period (usually 2–4 weeks). We can help manage the transition process to ensure continuity of your supports. Contact us to discuss how we'd handle the handover.
Xyston charges the NDIS published price limits — the maximum rates set by the NDIA for Support Coordination. These are reviewed annually and apply to all registered providers. Your coordinator will discuss your available budget and how we'll use it before any service begins.
Yes — participants in all management types (NDIA-managed, plan-managed, and self-managed) can access Support Coordination if it's funded in their plan. The management type affects how other supports are paid, but Support Coordination works the same way regardless.
Support coordinators are not a crisis or emergency service — in any life-threatening situation, call 000 immediately. For mental health crises, Lifeline (13 11 14) and your state's Community Mental Health Crisis Assessment and Treatment (CATT) team are the appropriate services. SIL providers and Behaviour Support Practitioners hold specific responsibilities for participants in crisis within their services. What Xyston does is work with you proactively — building a personalised crisis plan that identifies early warning signs, the right contacts to call, and a clear action plan before any crisis occurs. After an acute event, your coordinator will help stabilise your supports and assess whether your current plan is still meeting your needs.
