Coordination of Supports is the most widely funded form of NDIS Support Coordination. Your dedicated Xyston coordinator takes a genuinely hands-on role — working directly with your providers, solving problems as they arise, and steadily building your confidence to navigate the NDIS independently.

How it works

From first contact to active coordination — a simple, transparent process designed around you.

Submit a Referral

Complete our online pre-screening form or call us directly. We'll review your information and confirm whether we're the right fit — usually within two business days.

Initial Conversation

We'll reach out to the participant (or their representative) for an introductory call to understand their goals, situation, and what great coordination looks like for them.

Service Agreement

We'll prepare a clear Service Agreement outlining what we'll do, how often we'll connect, and how your Support Coordination budget will be used.

Active Coordination Begins

Your dedicated coordinator gets to work — connecting providers, tracking progress, monitoring your budget, and keeping your NDIS plan moving forward.
